The First Practice
In 2011 I built a traditional cataract practice from scratch in Denton, Texas. I did every office task myself in the early days, then replaced myself with staff as the practice grew. Over a decade the practice matured, and in the mid-2020s it was sold to private equity. That transaction taught me a lot about what a practice is really worth and what parts of it actually generate value.
The Rebuild
In 2024 I started a new practice. Same specialty, same city, different model. This time, instead of replacing myself with staff, I replaced each task with a computer automation. Fax intake, voicemail routing, patient scheduling, check-in, insurance verification, all handled without human touch. My role became to see one new patient per day, operate on four to five cataract cases per week, and handle every direct patient interaction personally. No front desk, no biller, no office manager. Just me and the automations.
The Model
The economics work because overhead disappears. The patient experience improves because I am the person they interact with directly. The clinical work is unchanged because AI does not touch clinical decisions. The pattern is repeatable, and I believe it represents a new form of small-office medical practice that other physicians can adopt.
That is what this course teaches.
